During the Open Enrollment period for 2018 benefits, you may purchase Supplemental Life Insurance coverage of one to eight times your annual base pay, up to a maximum of $3 million. You will need to provide Evidence of Insurability (EOI), or proof of good health, to MetLife to increase your coverage or initially purchase Supplemental Life Insurance coverage.

After you make an election requiring EOI, a link that will prompt you to complete the form electronically will appear under action items on the Benefits Enrollment site. If you do not enroll online within seven (7) days, a form will be sent to you automatically if your coverage election requires EOI.
